General Enrollment

Please fill out the following form to enroll your child at one of our Little Jewels locations. 

Your center director will contact you when they receive your child’s completed enrollment form.

We are so glad you have chosen to trust us with your child. Your child will soon begin their journey of learning and fun.

Please fill out the following form in its entirety and submit it at least two days prior to your child starting. In addition to the forms, we will need a copy of your child’s birth certificate from the county, current physical & immunization record, a copy of parent/guardian(s)’s driver’s licenses, and signed Parent Acknowledgment from our parent handbook.
